Special Issue Information

Dear Colleagues,

It is our pleasure to introduce to you this special edition of Children, aimed at addressing issues related to pediatric anesthesiology, surgery, pain management and the management of the perioperative process. There have been considerable developments in the field of anesthesiology and perioperative medicine in recent years, including pharmaceuticals, anesthetic technique, monitoring, surgical techniques and postoperative pain management. Most importantly, however, has been the design of surgical pathways, early recovery after surgery (ERAS) protocols, multimodal postoperative pain protocols and other complex multidisciplinary plans that have greatly enhanced perioperative care. Although anesthetic techniques and safety have diversified and improved, new data suggesting that there may be potential for some adverse long-acting effects of anesthetics, especially in younger and more medically fragile children, contribute to increased caution in the recommendation of elective procedures in those under 3 years of age.   

This edition seeks to provide a close-up view of several of these themes and aims to assist the pediatrician in becoming part of the perioperative team by helping the prospective patient and family to plan and navigate the complex waters of surgical intervention and hospitalization.

We are soliciting papers that will elucidate some of the topics below:

  • Common questions a pediatrician might get asked in the perioperative period;
  • Anesthesia and the developing brain;
  • NPO and sedation;
  • Anesthesia for the ex-premature baby;
  • Anesthesia for the obese patient;
  • Management of kidney and liver disease in the perioperative period;
  • Management of endocrine disorders in the perioperative period;
  • Multimodal analgesia;
  • Postoperative pain management strategies;
  • Nonpharmacological strategies in the perioperative period;
  • Perioperative acupuncture in the pediatric patient;
  • ERAS protocols;
  • Management of the patient undergoing scoliosis surgery;
  • Management of the patient undergoing NUSS procedure;
  • Management of the patient undergoing urologic procedures;
  • Optimization of the pediatric cardiac patient for non-cardiac surgery.

Effectiveness and Safety of Intravenous Sedation with Propofol in Non-Operating Room Anesthesia (NORA) for Dental Treatment in Uncooperative Paediatric Patients

Background: Uncooperative children require sedative approach for dental treatment. The aim was to assess the effectiveness of Propofol in “Non-Operating Room Anesthesia” (NORA) for paediatric dental treatment; intraoperative side effects; postoperative side effects; post-discharge effects. Methods: a prospective study, involving 109 uncooperative children undergoing sedation in NORA using Propofol for dental treatment, was performed. Working sessions, success/failure, intraoperative and postoperative side effects, number of treatment; type of procedure were assessed. Parents completed a post-discharge questionnaire on: pain; crying; fever; vomiting; headache; drowsiness; excitability; irritability; ability to eat; drugs and medical care needing. Results: Success: 96.7%. Intraoperative side effects: 33.3%. Postoperative side effects: 6.4%. Statistically significant association between: intraoperative side effects and age (p = 0.001), health status (p = 0.0007), weight (p = 0.038), respectively; intraoperative side effects and number/ type of dental treatment (p = 0.0055) and scaling (p = 0.0001), respectively. For post-discharge questionnaires, statistically significant association between: age and crying (p = 0.0001) and headache (p = 0.002), respectively; health status and crying (p = 0.015) and drugs needing (p = 0.04), respectively; weight and crying (p = 0.0004); extraction and pain (p = 0.0001) and crying (p= 0.0073), respectively; scaling and crying (p = 0.04), excitability and irritability (p = 0.03), respectively. Conclusion: Propofol in NORA was effective with minimal side effects. Full article
