Introduction: Chronic obstructive pulmonary disease is the most common pulmonary disease, mostly caused by smoking. Reduction of this habit in the society, should be one of the most important tasks for physicians. They should set a good example. The aim of the study was to assess smoking habits among Polish pulmonary physicians. Material and methods: During a congress of the Polish Respiratory Society in 2006 participants were asked to fill out an anonymous questionnaires including questions regarding age, sex, and smoking status. Results: Questionnaires were filled in by 406 physicians (43% of congress participants): 272 women (67%) and 134 men (33%). There were 10.1% current smokers (9% in women, 13% in men), 19.5% were ex-smokers (18% were women, 23% were men), and 70.2% never smokers (73% were women, 64% were men). Conclusions: Prevalence of smoking among Polish pulmonologist is decreasing. However, it is still higher than in countries with low smoking prevalence in the general population. Full article

Abstract
Samoistne śródmiąższowe zapalenia płuc to niejednorodna grupa chorób o podobnych objawach klinicznych. Samoistne śródmiąższowe zapalenia płuc to niejednorodna grupa chorób o podobnych objawach klinicznych. Podstawą ich obecnie stosowanej klasyfikacji są w głównej mierze różnice w obrazie histologicznym, jednak istotnym dopełnieniem całości procesu diagnostycznego, pomagającym w ustaleniu precyzyjnego rozpoznania, jest obraz tomograficzny o wysokiej rozdzielczości. Jedną z rzadziej występujących form samoistnego śródmiąższowego zapalenia płuc jest złuszczające śródmiąższowe zapalenie płuc występujące częściej u mężczyzn obciążonych wywiadem nikotynowym w 4.–5. dekadzie życia. W pracy przedstawiono przypadek 50-letniego mężczyzny przyjętego na Oddział Pneumonologii Śląskiego Uniwersytetu Medycznego w celu dalszej diagnostyki przewlekającego się obustronnego zapalenia płuc, z objawami upośledzonej tolerancji wysiłkowej, produktywnym kaszlem i stanami podgorączkowymi. Na podstawie wykonanych badań dodatkowych, zgodnych z zalecanym algorytmem postępowania diagnostycznego, obejmujących również wideotorakoskopię z pobraniem wycinków tkankowych, dokonano ostatecznego rozpoznania złuszczającego śródmiąższowego zapalenia płuc. Po zastosowaniu steroidoterapii systemowej obserwowano remisję choroby. Full article

Abstract
Ziarniniakowatość Wegnera jest chorobą układową o nieznanej etiologii charakteryzującą się zapaleniem naczyń z tworzeniem martwiczych ziarniniaków, które zajmują głównie górne i dolne drogi oddechowe oraz nerki. Ziarniniakowatość Wegnera jest chorobą układową o nieznanej etiologii charakteryzującą się zapaleniem naczyń z tworzeniem martwiczych ziarniniaków, które zajmują głównie górne i dolne drogi oddechowe oraz nerki. Przedstawiony przypadek dotyczy 44-letniej kobiety, u której wystąpiły nietypowe zaburzenia neurologiczne. U pacjentki z mnogimi cieniami okrągłymi i powiększonymi węzłami chłonnymi wnęki w RTG klatki piersiowej wystąpił nagły epizod niedowładu połowiczego. Tomografia komputerowa ujawniła ognisko niedokrwienne w mózgu. Rozpoznanie ziarniniakowatości Wegenera potwierdzono za pomocą badania histologicznego wycinków pobranych w trakcie biopsji otwartej płuca. Podczas pełnego kursu chemioterapii prednisolonem i cyklofosfamidem uzyskano zmniejszenie zaburzeń neurologicznych i znaczącą poprawę stanu klinicznego chorej. Full article

Devastating effects of smoking on the respiratory system are commonly acknowledged. However, detailed knowledge of the responsible mechanisms is insufficient to allow early diagnosis and effective treatment of the smoking-related pathologies. This paper presents a review of key processes occuring in the smokers’ lungs, particularly these responsible for the persistent inflammatory response and neoplastic transformation. Full article

Hundreds of millions of people of all ages suffer from chronic respiratory diseases which include asthma and respiratory allergies, chronic obstructive pulmonary disease, occupational lung diseases and pulmonary hypertension. More than 500 million patients live in developing countries or in deprived populations. Chronic respiratory diseases are increasing in prevalence. Although the cost of inaction is clear and unacceptable, chronic respiratory diseases and their risk factors receive insufficient attention from the healthcare community, government officials, media, patients and families. The Fifty-Third World Health Assembly recognised the enormous human suffering caused by chronic diseases and requested the World Health Organization (WHO) Director General to give priority to the prevention and control of chronic diseases, with special emphasis on developing countries. This led to the formation of the WHO Global Alliance against Chronic Respiratory Diseases (GARD). GARD is a voluntary alliance of organisations, institutions and agencies working towards a common vision to improve global lung health according to local needs. GARD is developed in a stepwise approach using the following three planning steps: estimate population need and advocate action; formulate and adopt policy; and identify policy implementation steps. This article presents a review of ultrasonographic methods and their usefulness in pulmonary diagnostics. Because of its advantages ultrasonography offers a quick, cheap, and safe diagnostics of variety of pulmonary diseases. The role of ultrasound in diagnostics of lung neoplasms, pleural, diaphragma, pericardium and vessels pathology was described. Some restrictions of this tool were mentioned. The newest methods of endobronchial ultrasound were also discussed. Full article

The Prevalence of Tobacco Smoking and Attitudes of Polish Pulmonologists towards Smoking

Introduction: Pulmonologists can play an important role in smoking prevention and control. The aim of this study was to assess smoking prevalence among Polish pulmonologists and physicians’ behaviour towards smoking patients. Material and methods: The study was performed in 2006 during national congress of Polish Respiratory Society. The survey was performed using a questionnaire including questions regarding age, sex, professional activities, specialty, academic degree and respondents’ nicotine status, attitude and activity towards smoking patients. Results: Questionnaires were filled in by 498 physicians: 160 (32.1%) men and 338 (67.9%) women, out of 852 participating physicians (58.5%) who received the questionnaire. There were 11.3% smokers (13.8% men and 10% women), 25.1% of surveyed physicians were ex-smokers, and 63.6% never smokers. 91.4% of respondents always ask patients if they smoke. 87.4% physicians always advise smoking cessation to their patients, but smoking cessation support is offered only by 48% of respondents. Almost half respondents use drugs in nicotine dependence treatment. Majority of respondents (81.1%) think that physician’s attitude influences patient’s behaviour. More than a half (55.7%) think that smoking by physicians is not in accordance with physician’s ethics. 91.4% support smoking ban in public places. Conclusions: This study found a lower prevalence of smoking among Polish pulmonologists compared with the general population, and with the previous surveys and demonstrated the impact of personal smoking on physicians’ attitudes towards smoking. Introduction: Evaluation of effectiveness of pulmonary rehabilitation in patients with idiopathic pulmonary fibrosis (IPF) has not yet been presented in medical literature. The objective of the study is to analyze the influence of inspiratory muscle training on dyspnea (oxygen cost diagram [OCD], baseline dyspnea index [BDI]), quality of life (SF-36), results of 6 MWT (distance, dyspnea in Borg’s scale), maximal inspiratory pressure (MIP), and lung function tests (IC, TLC, VC, FEV1, DLCOSB, DLCO/VA) in patients with IPF. Material and methods: Investigations were conducted before, after 6 and 12 weeks of pulmonary rehabilitation performed in 2 groups of patients: study group (GB)—16 patients—with inspiratory muscle training added to general body conditioning and in control group (GK)—14 patients—who performed only general body conditioning. Results: After 12 weeks of rehabilitation in SG we noticed the significant decrease of dyspnoea before (p = 0.028) and after (p = 0.012) 6 MWT, increase of distance in 6 MWT (p = 0.001), increase of MIP (p = 0.006), decrease of dyspnoea in BDI (p = 0.001) and improvement of quality of life (SF-36/PCS; p = 0.030) in comparison to baseline values. In the GK we observed increase of distance in 6MWT (p = 0.001) and improvement in quality of life (SF-36/PCS; p = 0.016). No improvement in sensation of dyspnea during 6MWT, BDI and MIP was noticed in the GK. Conclusion: Adding inspiratory muscle training increases effectiveness of pulmonary rehabilitation in IPF patients. Full article
