The objective of the study was to assess the safety of the HyperSound® Audio System (HSS), a novel audio system using ultrasound technology, in normal hearing subjects under normal use conditions; we considered preexposure and post-exposure test design. We investigated primary and secondary outcome measures: i) temporary threshold shift (TTS), defined as >10 dB shift in pure tone air conduction thresholds and/or a decrement in distortion product otoacoustic emissions (DPOAEs) >10 dB at two or more frequencies; ii) presence of new-onset otologic symptoms after exposure. Twenty adult subjects with normal hearing underwent a pre-exposure assessment (pure tone air conduction audiometry, tympanometry, DPOAEs and otologic symptoms questionnaire) followed by exposure to a 2-h movie with sound delivered through the HSS emitter followed by a post-exposure assessment. No TTS or new-onset otological symptoms were identified. HSS demonstrates excellent safety in normal hearing subjects under normal use conditions. Full article

This study aimed to quantify outcomes in a group of patients who were implanted with an Oticon Medical Neurelec (Vallauris, France) cochlear implant system, the Digisonic® SP/Saphyr® Neo. Ten participants took part in this preliminary study. Their speech perception capacities were evaluated at 3, 6, and 12-months after cochlear implant activation and compared to pre-implantation scores and to scores observed with former versions of the sound processor. Compared to former versions of the sound processor, patients using the Saphyr® Neo processor obtained better speech perception scores for sentences in silence at each tests session (3 months: 79%, 6 months: 82% and 12 months: 94%) compared to Digisonic® users (respectively: 58%, 69% and 75%) and Convex sound processor users (resp. 39%, 59% and 51%). These observations confirm that the technological improvements made in the Saphyr® Neo sound processor coupled with the Digisonic® implant, provided quantifiable benefits in speech perception in Quiet compared to former versions of the processor Convex and Digisonic® SP. The Nucleus® 5 System Sound Processor (CP810, Cochlear, Macquarie University, NSW, Australia) contains two omnidirectional microphones. They can be configured as a fixed directional microphone combination (called Zoom) or as an adaptive beamformer (called Beam), which adjusts the directivity continuously to maximally reduce the interfering noise. Initial evaluation studies with the CP810 had compared performance and usability of the new processor in comparison with the Freedom Sound Processor (Cochlear) for speech in quiet and noise for a subset of the processing options. This study compares the two processing options suggested to be used in noisy environments, Zoom and Beam, for various sound field conditions using a standardized speech in noise matrix test (Oldenburg sentences test). Nine German-speaking subjects who previously had been using the Freedom speech processor and subsequently were upgraded to the CP810 device participated in this series of additional evaluation tests. The speech reception threshold (SRT for 50% speech intelligibility in noise) was determined using sentences presented via loudspeaker at 65 dB SPL in front of the listener and noise presented either via the same loudspeaker (S0N0) or at 90 degrees at either the ear with the sound processor (S0NCI+) or the opposite unaided ear (S0NCI-). The fourth noise condition consisted of three uncorrelated noise sources placed at 90, 180 and 270 degrees. The noise level was adjusted through an adaptive procedure to yield a signal to noise ratio where 50% of the words in the sentences were correctly understood. In spatially separated speech and noise conditions both Zoom and Beam could improve the SRT significantly. For single noise sources, either ipsilateral or contralateral to the cochlear implant sound processor, average improvements with Beam of 12.9 and 7.9 dB in SRT were found. The average SRT of –8 dB for Beam in the diffuse noise condition (uncorrelated noise from both sides and back) is truly remarkable and comparable to the performance of normal hearing listeners in the same test environment. The static directivity (Zoom) option in the diffuse noise condition still provides a significant benefit of 5.9 dB in comparison with the standard omnidirectional microphone setting. These results indicate that CI recipients may improve their speech recognition in noisy environments significantly using these directional microphone-processing options. Full article

One of the recent diagnostic tests to assess the function of otolithic organs is through vestibular evoked myogenic potential (VEMP) testing. There are equivocal findings on effect of aging on ocular VEMP (oVEMP) parameters with reference to latencies. Hence this study was taken up to investigate the age related changes in oVEMP parameters. This present study considered 30 participants in each age group i.e., young adults, middle-aged adults and older adults. oVEMP were recorded using insert earphone at 100dBnHL at 500hZ short duration tone burst. The results showed in older adult significant difference in response rate, latencies and amplitude as compared to young and middle adult. Hence age should be taken into consideration when interpreting oVEMP results. Full article

This paper presents the configuration and digital signal processing details of a tablet-based hearing aid transmitting wirelessly to standard earphones, whereby the tablet performs full sound processing rather than solely providing a means of setting adjustment by streaming to conventional digital hearing aids. The presented device confirms the recognized advantages of this tablet-based approach (e.g., in relation to cost, frequency domain processing, amplification range, versatility of functionality, component battery rechargeability), and flags the future wider-spread availability of such hearing solutions within mainstream healthcare. The use of a relatively high sampling frequency was found to be beneficial for device performance, while the use of optional off-the-shelf add-on components (e.g., data acquisition device, high fidelity microphone, compact wireless transmitter/ receiver, wired headphones) are also discussed in relation to performance optimization. The easy-to-follow configuration utilized is well suited to student learning/research instrumentation projects within the health and biomedical sciences. In this latter regard, the presented device was pedagogically integrated into a flipped classroom approach for the teaching of bioinstrumentation within an Allied Health Sciences School, with the subsequent establishment of positive student engagement outcomes. Individuals with unilateral hearing loss of severe-profound degree face listening difficulties while localizing a sound source and while perceiving speech in the presence of noise. The objective was to compare the efficacy of the digitally programmable bone anchored hearing aid (BAHA), trimmer digital BAHA and the transcranial contralateral routing of signal (T-CROS) in improving the listening performance in adults with unilateral hearing loss. Twenty-four adults with unilateral hearing loss was assessed for sound field thresholds, speech perception performance in quiet and noise (direct and indirect conditions) and the subjective quality rating of speech in unaided and aided with either T-CROS or digitally programmable BAHA or trimmer digital BAHA attached to the headband. Results indicated that the participants performed better with both the digitally programmable and the trimmer digital BAHA than the T-CROS in both quiet and noise. However, the digitally programmable BAHA performed better when the speech arrived from the poorer ear side. The current study helps in prioritizing the hearing amplification devices for the trial and also helps in arriving at the appropriate hearing amplification device for the individuals with unilateral hearing loss. Dealing with environmental noises presents a major issue for cochlear implant (CI) users. Hence, digital noise reduction (DNR) schemes have become important features of CI systems. Many noises like for example clinking glasses or slamming doors, have impulsive onsets and decay quickly. Common DNR algorithms cannot handle this type of noise in an appropriate way. In this study, we investigated the effect of an algorithm specially designed for such noises with 12 CI users (age range: 45 to 75 years). Speech scores in noise and quiet as well as subjective ratings of speech clarity, comfort and overall preference were measured. The main finding was a significant improvement of up to 1.7 dB of the speech reception threshold in noise as well as increased speech clarity. Speech in quiet was not negatively affected by the algorithm. The study revealed that the tested algorithm has the potential to improve CI listening. However, further research is needed regarding the effectiveness and suitability of the algorithm in daily use. Full article
