I read "Systemic action of inflammatory mediators in patients with essential hypertension and diastolic chronic heart failure: clinical pathophysiological study" with great intres, nonetheless I have some concerns:
1) This Journal has world wide range, please use European Society of Cardiology (ESC) or American Heart Association (AHA) classifications and definitions.
2) Please define degrees of heart failure? New York Heart Association scale (NYHA) is used worldwide. Please convert.
3) I can not understand study flow. Is this prospective study or retrospective registry study? Please add figure with study flow. One picture is wotrth more than 1000 words.
4) In exlusion criteria I can see valvular heart disease- please define. Mild MR or TR disqualifies patients form the study?
5) Please clarify how many female subjects were in post- menopausal. This is quite crucial for the study (10.3390/jcm8060792)
5) In statistical analysis I can not find do Authors used non-paremtric test for skewed distribution?
6) Dissusion is very long. I suggest to make it shorter, more condense and easer to read.
7) I can not find any limitation of you study. Please add paragraph.

The aim of the study, as stated by the authors, was to establish a correlation relationship between indicators of pro-inflammatory status and the structural and functional characteristics of the cardiovascular system in patients with essencial arterial hypertension with diastolic complications as well as the differevce between genders.
The study would be strengthened by picture from echocardiograph such as trace of left ventricle, the trace from a pulse tissue Doppler sample placed at the valve or so.
Another minor question is about statistics. What kind of post-hoc test did authors use?
Did authors see functional differences between patients with 1st and 2nd degree of hypertension? If yes, should be stated.
