Pharmacists are important members of the interdisciplinary teams in palliative care. Their unique expertise in management of medicines and formulation complements the skills of other health professional in the team. The role of the pharmacist has changed dramatically over the past 30 years and extends beyond the supply of the many different medications often required by patients. Pharmacists assume the responsibility and accountability for managing medication therapy, improving the quality of patient care by optimising the use of medicines and rationalising expenditure. It encompasses medicines reconciliation, a process that is essential in ensuring accurate and reliable information about the patient’s medication is transferred between care settings. Pharmacists have numerous other roles, including counselling patients and carers on the use of medicines, education of health professionals, provision of pharmaceutical and therapeutic advice and clinical research, the outcomes of which will influence day to day clinical practice. Therefore, this special issue aims to highlight the contributions of pharmacists in palliative care and provide a way for different models of practice or delivery of education to be reported. In this Special Issue, research articles, review articles around pharmacy and palliative care and reports on novel approaches being used by the interdisciplinary team to enhance patient care will be encouraged.

Manuscripts can be submitted until the deadline. All submissions that pass pre-check are peer-reviewed. Accepted papers will be published continuously in the journal (as soon as accepted) and will be listed together on the special issue website. Research articles, review articles as well as short communications are invited. For planned papers, a title and short abstract (about 100 words) can be sent to the Editorial Office for announcement on this website.
Submitted manuscripts should not have been published previously, nor be under consideration for publication elsewhere (except conference proceedings papers). All manuscripts are thoroughly refereed through a single-blind peer-review process. Pharmacy is an international peer-reviewed open access semimonthly journal published by MDPI.
