Dear Colleagues,

The introduction of Ni-Ti Rotary instruments has deeply changed the approach to and the results of endodontic treatments. Despite several discoveries being obtained through the years, several things are still to be understood about Ni-Ti files and their behaviors while rotating inside the root canal. Indeed, it is not infrequent Ni-Ti instruments failure, such as untwisting or even separation. Moreover, it is still controversial both the influence of the different in vitro studies on clinical performances and the relationship between the different properties and their influences during instrumentation. The aim of the current Special Issue is to increase the knowledge about the in vitro and in vivo performances of rotary files, with the scope of increasing the safety of root canal therapy thanks to more efficient instruments and instrumentation techniques. With these premises, the need for better and more specified studies about the Ni-Ti rotary instruments in vitro and in vivo behavior is arising day after day, mainly but not only focused on better specifying the relationship between the different well-known characteristics of rotary files as well as the in vitro evaluation of the multitude of new files recently introduced on the marketplace. Last but not least, the interaction between shaping technique and both cleanness effectiveness and tooth resistance.

Cleaning and Disinfecting Oval-Shaped Root Canals: Ex Vivo Evaluation of Three Rotary Instrumentation Systems with Passive Ultrasonic Irrigation

Background and Objectives: Successful root canal treatment depends on the thorough removal of biofilms through chemomechanical preparation. This study aimed to investigate and compare the cleaning and disinfecting efficiency of oval-shaped root canals using XP-endo Shaper (XPS), ProTaper Next (PTN), and HyFlex CM (HCM) in combination with passive ultrasonic irrigation (PUI). Materials and Methods: Ninety extracted teeth were contaminated and randomly divided into three groups: XPS, PTN, and HCM. Each group was assigned to three subgroups: subgroup A (sterile saline), subgroup B (3% sodium hypochlorite and 17% ethylenediaminetetraacetic acid), and subgroup C (3% sodium hypochlorite, 17% ethylenediaminetetraacetic acid, and PUI). Bacterial sampling was conducted both from baseline samples and samples after chemomechanical preparation. Scanning electron microscopy (SEM) was used to evaluate the residue bacterial biofilms, hard tissue debris, and smear layers on the buccolingual walls of oval-shaped root canals. Results: When combined with sterile saline, XPS demonstrated a higher reduction of bacterial counts and was more effective in eradicating Enterococcus faecalis in the middle third of the canals compared to the other instruments (p < 0.05). Additionally, when used with antimicrobial irrigants, XPS was more effective in disinfecting the coronal third of the canals than the other instruments (p < 0.05). Furthermore, XPS reduced hard tissue debris more effectively in the middle third of canals than in the apical third (p < 0.05). Conclusions: XPS outperforms PTN and HCM in disinfecting oval-shaped root canals. Despite the fact that combining XPS and PUI improves cleaning and disinfecting, removing hard tissue debris from the critical apical area remains challenging. Full article

Introduction The purpose of this research is to assess the wearing of the rotary file system (Protaper Next) and reciprocating file systems (Reciproc Blue and WaveOne Gold) at different lengths using a novel technique after in vivo clinical use. Materials and Methods Twelve different unused samples from each brand were accepted as reference values. For three different brands, the diameters of the files were measured by taking 12 samples used once, 12 samples used twice, and 12 samples used three times. Images were taken with a USB Micron Microscope, and file diameters were measured by determining limit values with Autocad. Result Reciproc Blue system was the most worn at apical 1 mm, and WaveOne Gold system was the most worn at apical 3 mm. PTN system exhibited the least wearing at any length. Moreover, less wearing was observed in the rotation motion than in the reciprocating motion. Conclusion In clinical practice, for the guttapercha to be fully adapted to the apical construction prepared according to the determined WL, the file should not undergo any wearing and volume reduction. Wearing—especially in the apical parts of the file—causes less preparation, and this situation could lead to apically obturation failure. Full article
